Staffing Shifts To Consider
You may direct the analysis to consider 8 hour shifts only, 8 and 10 hours
shifts, or 8, 10, and 13 hour shifts. In addition, if you want to do a cost comparison
among these three scenarios, you can select all three shifts and perform a statistical
analysis of the cost differences.
Relative Cost
The value here is taken from the relative cost associated with the dataset, but
you can enter a different value here. This allows you to rerun the analysis with different
relative costs without having to return to the dataset screen. The relative cost is the
ratio of the cost one over-utilized hour to the cost of one underutilized hour. The
assumption made is that the cost of one under-utilized hour is the actual cost to staff
that hour. An under-utilized hour is an hour of time assigned to a surgical service during
which no case was performed. An over-utilized hour is an hour of a case performed after
the end of regularly scheduled hours (e.g., after 3 pm).
Apply Service Constraints
Checking this box will apply any service constraints entered. A service
constraint specified that a given service must be assigned one or more rooms, regardless
of the results of the first shift analysis. This may be a political decision, or one based
on contractual arrangements between the facility and a surgical service.
Always Assign Urgent Room
An "Urgent" service is defined to handle allocating space cases that
are designated as being of an urgent (i.e., non-elective) nature. If there are too few
urgent cases to warrant assigning a designated room or rooms for such cases, then these
cases are combined in the "Other" service. However, if a facility is a trauma
center, at least one urgent room may need to be assigned each day to guarantee that a room
will be available. Checking this box will force assignment of an urgent room every day of
the week. Otherwise, the urgent rooms may be combined with the Other, depending on the
volume of cases in this group.
Ask About 2nd Shift Staff
In assigning rooms, the 1st shift analysis ignores the presence of 2nd shift
staff that may be present to perform elective and urgent cases. In that the presence of
such staff will affect the hours calculated as being over-utilized, checking this box will
result in the user being prompted to enter the number of staff on 2nd shift. The
percentage of the 1st shift costs that need to be reduced is then calculated and
displayed. If there are many 2nd shift staff (many of whom may turn out not to be
necessary.
